Please print out the first three forms listed below (Registration Form, Professional Services Agreement, and Policies and Practices for Private Health care Information), complete them and bring them with you to your first appointment. Two of them only require a signature on the last page. You should also print out the fourth form (Directions) if you would like help locating my office.
